However, not all of these patients lived in the community but in f.e. assisted living facilities. These patients as well as terminal ill patients, although still present in the electronic medical records of the general practice, were not approached as the program focused on community dwelling older people. Furthermore, not all frail patients received a CGA at home. The reason was that frail patients refused to the CGA at home at the time the practice or district nurse called these patients to make an appointment
