  1. Cox Regression analyses of the baseline and follow up. Calculations were based on monthly follow ups for nine years from the baseline interview (or seven and a half years from the 18 month follow up). The frailty index hazard ratios (HR) are calculated with % levels of the index (i.e. the HR measures a change of 0.01 on the index).
